## 1. Preserve the claim as received

- Record ID and version:
- Date and time (UTC):
- Original prompt or task, with private information removed:
- Exact claim as written (do not paraphrase):
- Product, venue, instrument, market and account mode named:
- Claim type: venue mechanic / calculation / historical result / product behavior / law or regulation / other:
- Which words imply certainty, universality, prediction or a guarantee?

## 2. Define the evidence required

- What observation would prove or disprove the exact wording?
- Primary source or raw dataset URL / identifier:
- Source owner and authority for this claim:
- Publication or effective date, retrieval timestamp (UTC) and version:
- Jurisdiction, venue, contract, product, account mode and time period in scope:
- Does this evidence support the claim, only a narrower statement, or neither?
- What source, observation or scope detail is still missing?

## 3. Reproduce calculations or behavior independently

- Formula, code version or exact reproduction steps:
- Input values, units, timestamps, assumptions and data fingerprint:
- Hand-check or independent implementation:
- Boundary cases checked (zero, missing, stale, negative or partial where relevant):
- Expected result and observed result:
- Difference, rounding, execution or data limitations:
- Evidence artifact ID; do not store credentials, API secrets or personal data:

## 4. Decide and rewrite narrowly

- Decision: keep within stated scope / revise / reject / unresolved / qualified review required:

## 5. Keep correction history

Append corrections instead of replacing the original claim. If evidence is unavailable, keep the status unresolved and name the next verification step. A reproducible calculation can still reproduce a bad premise; verify inputs and scope independently.
